The game gets too long and you have little influence on the size of the cathedrqls. Some tiles clearly have a building over the road so they do break it, others don’t and we play it that this doesn’t.
If ruoes complete a city with a Cathedral in it then you get three points per tile and shield. Finally, there are 6 point tiles to make scoring easier when players score more than 50 points. I’m going to get a bit less verbose from here on in, because for me, the “ideal” Carcassonne setup is Inns, Cathedrals, Traders and Builders, with or without the River.
Upvoted for not only being an excellent answer, but also restricting yourself to the three BEST Carcassonne expansions. It doesn’t go to the edge of the tile, so just like with a monastery or road ending bush, farms go around it.
My friends like this and as such I play this expansion very often. This exception is removed from the third edition, in which there is no difference between two-tile cities and cities of larger size.

If the road were meant to be broken on that tile, it would have the cathedraps of town in brown dirt like the tile on the left. To summarise, if you want a sixth player option then this is great and it does spice up the game if you are going to play Carcassonne a lot. Like a normal meeple, it can be played as a cathedralz on roads, a knight on cities, a farmer on fields, or a monk on monasteries.
